Lifetime runs for each major release. Don't worry about v4 it won't be out until at least late 2018.

No, everybody who wants to upgrade will need to pay. The pricing, discounts and whatnot is exactly the same for every client regardless if you got your license directly or via TMDHosting.

Upgrade isn't mandatory, you do it because you want/need more features, not to keep using what you already paid for.
