Error Messages on Upload

Just found out from a couple of the members of my site that they occasionally get an error message during uploads. Apparently it's been going on a while and repeating the upload a couple times fixes the problem but one guy had to try 4 times before he was able to upload.
He sent me these screenshots:

Any idea what the problem could be?
