404 Not found on install

When I install chevereto (latest version), I get a 404 (The requested URL /image/install was not found on this server.) after installing all dependencies.
This occurs regardless of using web installer or direct download.
I've tried with several machines (Debian 8.0/Ubuntu 16.04.1). File permissions are set correct.
 
mod_rewrite is enabled. If I disable it, chevereto returns the following error:

The system has encountered errors that must be fixed to allow proper Chevereto functionality. Chevereto won't work until the following issues are solved:

Apache mod_rewrite is not enabled in this server. This must be enabled to run Chevereto.
