Description of the issue
I have a user who previously uploaded a file. She deleted this file, altered the image, and uploaded the new file. When she uploads it, it shows the correct file in the thumbnail preview, but when the upload is done, it shows the original file that she deleted. How to resolve? I am sure she could rename the file, and it would solve the issue, but how do we fix the root problem? Thank you.
 
Show me the files, I can't help you without that. I need to check on my own whats going on.
