Visiting a site, with your own domain

Hey.. Do anyone know where i can get a script like this;

As you can see, hes having that URL to visit a other site from hes own site..

after domain: mydomain.com/url.php?url=yourothersite.com


Do anyone know how i could make this? And put like a bar ontop, that says somthing? Click at the demo url, and you'll see wht i mean 😛

What color/size, etc... do you want on your top bar?
