Disable Hotlinking for large images

Is it possible to disable hotlinking for the actual (large) images? And only allow thumbnails to be hotlinked?

Yes, make rules for all. Except for files ending with:
domain.com/images/xxx/image.th.png
domain.com/images/xxx/image.md.png
