Add description forms for images

Hello, it's possible to add new description forms for images?...

For example:
The image have: Title /Name Of Album/ Description
In dashboard there could be an option to add a new form like Place... or for my website (aerospotter.net) something like Aircraft/ Airline Name/ Aircraft Registration...

Can someone explain me how to do it please?
